I concur with this being the top differential. My greater concern is
that the patient's name (I suppose) was included on the image file. For
confidentiality reasons I suggest keeping patient information
confidential/anonymous in future. Just a thought, AGF

Shape, location, internal characteristics and (especially) the rough
surface all make me think of calcified lymph node.
