Many years ago a case was published in OOOOE (or whatever it was called then) of a case where one maxillary sinus was so large it crossed the midline and went half way over to the other side. The patient was the wife of one of our faculty members here at Michigan. This was in the days before panoramic radiographs were common. They had periapicals and a maxillary occlusal, which showed a gigantic radiolucency in the palate. She also had a couple heavily restored teeth and it was thought originally that this was a huge periapical lesion so endo was done. No better. Then a biopsy was done: normal respiratory epithelium. Finally a panoramic was done to show the aberrant sinus configuration.

Dear OradFriends,
In attachment an anatomical question for you. As you can see this patient has bilateral bulging maxillary antra, which come together at the midline under the floor of the nose. I have never seen such thing. Have you?
This scan was made for implantology purposes.
Kind regards from a white Belgium,
Happy New Year to you all!!
Johan
