I am thinking cyst of some sort. I have seen cysts displace the mandibular canal like this one does. I hope they are pointing out the caries on the maxillary molars.

A 48 years old male patient came to the clinic with a swelling in the right side of the mandible , the Panoramic view shows a corticated unilocular scaluped radiolucency in the premolar area displacing between the premolar and canine, clinically the swelling perforated the buccal cortical bone , aspiration was done from this area revealed yellowish-grayish material
Differential Diagnosis
1. Odontogenic keratocyst
2.Lateral periodontal cyst
3.Odontogenic myxoma
