More on the Murphy allegations of speech-basedplacementonno-fly list, and...

It  might be helpful if someone would explain what sort of  "incompetence" 
explains Professor Murphy's ordeal? Was there some sort of  government document 
(list) that an employee misread?  Was it another  "Walter F. Murphy"? Was 
Professor Murphy's name was on the government list,  but the employee didn't 
understand the appropriate response to someone whose  name was on the list?  Was 
the list not a government document at all, but  American Airline's own list?  
I'm trying to figure out how employee errors  might account for this incident.
