Turning Data into Action (TDA), a CDC REACH CORE Project, is gathering information on the local public health system in regards to air quality related health issues. In collaboration with the UCLA Center for Health Policy Research we are using the CDC Local Public Health System Assessment Survey in order to collect this information.

The Local Public Health System Assessment (LPHSA) Survey is a brief survey that aims to examine 10 Essential Public Health Services from the framework of air quality and asthma, cardiovascular disease, and low birth weight in Boyle Heights and Long Beach.

Who Should Complete the Survey?

TDA is asking all stakeholders to SHARE YOUR VOICE about the local public health system in your community. The public health system is not just the local health department. If you are a consumer of health services or a provider of services, you are the public health system. This includes:

• Health department staff working in the Los Angeles and Long Beach areas
• Health professional and primary care providers, such as physicians, nurses, front line staff, and volunteers
• Community members and healthcare users living in and around Boyle Heights and Long Beach
• Other individuals working in the community, such as schools and non-profits

Subscribing to Uclahealthdata
